US Clinical Fellowship
Please refer to the SFMatch website to register for the match and read about the new process for applying to Ophthalmology Fellowship training in 2008 for fellowships beginning July 2009. Once registered you can view our program listing on the SFMatch directory website for all required documents.
In Addition to what is listed on the SFMatch website please submit:
1. A wallet sized photo of yourself
2. A copy of your most current CV
3. A copy of your OKAP scores (a photo copy is acceptable)
These items can be sent by email to kortiz@eyecenter.ucsd.edu, or can be mailed to our address listed below. Please feel free to send any other documents that may enhance your application.
